Students may request access to records maintained by the School of Rehabilitation Science (SRS), including documents contained within their student file.
To submit a request, please complete the Student Records Request Form. Once your request has been received, an SRS representative will contact you to confirm the records being requested and discuss the process and timeline for providing access.
Please note:
- Requests may be subject to verification of identity before records are released.
- Some records may be withheld or redacted in accordance with applicable privacy legislation and University policies.
- Records will be provided only to the student unless written authorization is provided permitting release to another individual.
- Information collected through this form will be used solely for the purpose of processing your request and will be handled in accordance with University privacy and records management requirements.
