A form entitled "Authorization to Release Application and Admission-related Information" is available online for students' use. This form is used if the student requires authorization for release of information.
These forms will be kept on file and in a database at Student Central.
If a staff or faculty member encounter someone stating they have authorization for a release of student information, please call Student Central at (306) 966-1212 or email at askus@usask.ca for verification of authorization.
